This role will be based in Liverpool.
An exciting opportunity has arisen to join our Step Forward Team as a Mental Health Lead working across the Mental Health Care Division footprint. The posts will be based on a neighbourhood level, delivering a population based approach to care focusing on prevention and early intervention for mental health.
We are looking to recruit x 1 Permanent Band 7 Mental Health Practitioner to join the Step Forward Team and be part of and lead the development of the service. Using senior level leadership and clinical skills you will input to provide seamless, integrated, responsive and holistic care throughout each service users individual journey.
The role will focus on the interface between Primary and Secondary care, adopting a collaborative care model to ensure that we provide access to rapid evidence-based clinical input and specialist clinical support when appropriate.
Main duties of the job
You will be working in primary care settings working with specific PCN's, Integrated Care Teams and with Community Mental Health Teams /other specialist teams.
The post requires working autonomously and as part of a team. Resilience and adaptability in approach are expected as well as the ability to offer and receive support from colleagues within the team. You will be expected to spend time developing good working relationships with colleagues across Primary and secondary care, VCSFE and local networks to facilitate collaborative integrated working.
This role requires senior clinical and operational management skills and experience.
Clinical oversight and leadership skills are necessary for the role.
