Job summary
We are looking for Band 5 nurses to work within our district nursing teams based in Knowsley, to cover our 8am-8pm 7 days a week model, with vacancies available across the borough. The service is undergoing significant developments as part of our plan to deliver integrated community care for patients on a neighbourhood footprint across Knowsley.
Working to the Mersey Care values, you will have excellent empathy, communication skills and a person centred approach. You will be keen to continue learning and develop the competencies needed to give evidence based care to your patients in their own home. Clinical decision making skills and a problem solving approach is an essential component of this role. You will be a good team player who can also work autonomously on your own, escalating and reporting back appropriately.
The service operates 24 hours per day, 365 days per year to cover our 8am-8pm 7 days a week model.
You will also have the opportunity of a preceptorship and full support will be given to develop and enhance your skills.
Access to a vehicle is required for this role - being able to travel between multiple sites in the course of performing duties is essential. The role will involve regular travel across healthcare organisations sites in the area of Knowsley. Therefore, the post holder must be able to meet the travel requirements for this post.
Main duties of the job
The post holder is responsible for the delivery of clinical care to a defined population. Supported by senior practitioners, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
