Job summary

The post is a permanent position in the St Helens and Knowsley Community Learning Disability Team. The post holder will work flexibly to support the needs of the service, across the Intensive Support Function and the Community Learning Disability Nursing Team.
The post holder must have the means to travel within and outside of the boroughs of St Helens and Knowsley as part of the role.
This post is subject to an enhanced check with adults barred listDisclosure and Barring Service (DBS) Check.
Main duties of the job
The post holder will work collaboratively within a multi-disciplinary team to offer specialist nursing care which is holistic and person-centred ensuring continuity of care for service users on their journey through the St Helens and Knowsley Specialist Learning Disability Service. The post holder will work from within the community service but be expected to be flexible in terms of delivering care within a variety of settings.
The post holder will demonstrate an attitude which respects and values service users and their carers.
