Senior Nurse Practitioner

Job responsibilities

Clinical:

Provide specialist nursing assessments and care for adults with a learning disability.

Offer core holistic assessments included in the MDT processes and protocols.

Be competent in the core service offer of: Positive Behaviour Support; Transforming Care related crisis management; Frameworks of CPA, MHA, MCA; Physical Health Facilitation; Dementia; End of Life Care, Mental health and Mental Well-being. Utilise: Individual and Group Therapy; Enabling others to Support Systemic Care; Teaching; Multi professional and Multi Agency Consultation.

Assist in the development and implementation of integrated care pathways, including appropriate service user referrals and models of care.

Give/receive regular clinical supervision and case supervision as per Trust policy to ensure standards of service user care and documentation.

Ensure the clinical environment is conducive to promote service users health, safety and well-being, thereby preserving and respecting each service users personal dignity, privacy, religious and cultural beliefs.

Ensure record keeping is in accordance with professional and Trust standards.

Work in a professional and collaborative manner with the multi-disciplinary team.

Participate and promote positive intra-agency working.

Participate in the referral process, including the assessment of such individuals in accordance with the agreed care pathway and the completion of quality reports where appropriate.

Responsible for the assessment of care needs, and in partnership with the client and other significant others develop care plans, risk plans, relapse plans; implement and evaluate outcome of care for positive benefits for the service users physical and mental well-being.

Participate and assist in setting standards of service user care based on sound research findings.

Promote the principles of Living Life Well: self-management; person centred care; shared decision making; strengths based; collaborative co-production.

Participate in the triage rota

Provide effective leadership as requested and be a positive role model within the team.

Managerial:

Undertake delegated responsibilities as agreed with Team Manager/Clinical Lead

Support the formulation and implementation of clinical and operational practices and procedures as appropriate.

Participate in the recruitment, selection and induction of new staff.

Support the implementation, monitoring and updating of operational procedures and philosophy of care for the clinical area.

Ensure adherence and implementation of Mental Capacity Act as necessary.

Ensure the principles of Clinical Governance are incorporated in service delivery by maintaining a high quality service based on agreed clinical standards.

Ensure that service users are cared for within the confines of their respective Mental Health Act status and adhere to the requirements of the MHA 1983 and the corresponding Code of Practice. This may include liaison with the home office for restricted service users.

Ensure adherence and implementation of Care Co-ordination.

Ensure that Trust policies and procedures are adhered to.

Assist in the development of local protocols to ensure a high standard of service delivery is achieved and maintained.

Support the process of staff participation in the formulation of new ideas/suggestions to improve service user care and service delivery.

Provide statistical information as required.

Assist in the maintenance and updating of the units equipment inventories and service schedules. Ensure regular servicing, maintenance, repair or replacement as required adhering to Health & Safety, COSHH and Medical Devices Agency regulations.

Ensure effective processes of communication are established, maintained and regularly evaluated within designated team.

Assist in the investigation of complaints and incidents at team level in accordance with Trust policy.

Participate in Serious Untoward Incident investigations/reviews as required.

Provide relevant staff with the necessary management supervision regarding the co-ordination and delegation of duties, to ensure a comprehensive and consistent delivery of care is maintained.

Ensure that the nursing practice of the clinical area adheres to the Trusts policies and procedures.

Participate in the clinical supervision process.

Appropriate participation in managing performance of staff as required under supervision from team manager.

Qualifications

Essential

• Registered nurse qualification - RNLD
• Evidence of clinical experience in either challenging behaviour or mental health.
• Evidence of working in a range of service environments.
• Eclectic range of clinical competency.

Desirable

• Working with offenders.
• Working in mental health services.

KNOWLEDGE/ EXPERIENCE:

Essential

• Experience of Person-Centred Planning.
• Experience of Care Programme Approach.
• Experience in the development and implementation of complex care packages.
• Working knowledge and understanding of the Mental Health Act.
