Job summary
The nursing team consists of band 5 and band 6 nurse advisors who triage calls and referrals from many service lines. We process all cases on Adastra and EMIS systems .
Nurse Advisor - This is a permanent position (37.5 hrs per week).
We interact with patients , relatives and other health care professionals including secondary care.
We clinically triageunplanned calls and are driven by performance supported by policies and processes in a call centre environment
Flexibility is essential as the Nursing Team operates Monday to Sunday including bank holidays between the hours of 8am and 8pm. The successful candidate must be able to work until 8 pm as required.
Main duties of the job
The role involves working as part of a clinical team that provides clinical support to numerous clinical services, including District Nursing, Palliative Care service, Intermediate Care and Primary and Secondary Care services within Liverpool, Sefton and Knowsley. Suitable applicants will have excellent communication and IT skills. Training will be provided.
The nurses to operate within a call centre environment providing clinical assessment and advice to patients and carers liaising with other health care professionals including GPs to ensure the patient accesses the most appropriate clinical pathway and supporting admission avoidance.
