Job responsibilities
CLINICAL
The post holders will work alongside other staff to:
Promote excellence in clinical care by ensuring that practice is evidence based.
Contribute to the development, implementation and review of policies, protocols and guidelines in line with local and national policy.
Engage with and motivate colleagues and peers.
Work collaboratively at a clinical level to support practitioners in their practice.
Contribute to the induction of new starters within Looked after Children health and social care staff.
Act as an expert resource for advice, information, guidance and support for staff within health and social care.
Consult on complex issues in relation to looked after children, care leavers and unaccompanied asylum seeking children in formal settings.
Work effectively within the team as well as within the wider organisation.
Prioritise, organise and carry out own work effectively.
Actively promote quality in all areas of work across health and social care.
Implement, promote and maintain quality and governance systems and processes across health and social care.
Carry out individual health assessments in order to address the complex health needs of children in care. Conduct statutory Looked after Children review health assessments which includes developmental and emotional wellbeing of looked after children in accordance with national and local guidance and policies. This may be in home, clinic or other setting and may occasionally require the post holder to cross boundaries in order to undertake assessments up to 20 miles radius.
Carry and manage a clinical caseload of approximately 85 children and young people.
Provide additional support and brief interventions to children and young people including for example mental health interventions as part of the THRIVE model, sexual health interventions, immunisations, smoking cessation.
Provide health advice, support and training to professionals from other disciplines including foster carers on individual and general health issues to promote the health of looked after children and offer health promotion information regarding childhood issues.
Participate in clinical supervision and the Personal Development Process, establishing a personal development plan in line with Post Registration and Education and Practice, and to meet NMC requirements for re-validation.
Provide advice on health lifestyles and positive behaviour for looked after children, care leavers and unaccompanied asylum seeking children.
Promote good practice and standards of excellence at all times.
Maintain client/ patient confidentiality at all times unless in a safeguarding issue.
Effectively engage with hard to reach groups, offering outreach and creative methods to promote positive health outcomes for Looked after Children acting as a role model and advocate.
Operate an assertive outreach model with identified vulnerable clients; engaging them with a range of home or residential care home visits ensuring continued engagement.
Attend appropriate multi-agency meetings such as Looked after Children Reviews/MACE/Strategy meetings.
Attendance at Operational CSE/CE/Missing meetings as appropriate and liaison across health to inform of the risks to Looked After Children.
MANAGERIAL
The post holder will:
Influence and contribute to the development of the speciality by demonstrating the ability to keep up to date with current and emerging health and social care agenda and the wider childrens agenda.
Act as a resource for information, advice and support for children, young people, carers and other professionals within mainstream services.
Ensure practice is evidence based and delivered to an agreed standard in line with governance and quality frameworks.
Contribute to the development, implementation and evaluation of training programmes, in conjunction with Learning and Development.
Undertake audits and disseminate findings.
Contribute to networks locally, regionally and nationally and establish links.
Work in partnership with the Named Nurse for Looked After Children and Designated Professionals for Looked After Children to ensure a seamless service.
Report and contribute on the investigation process of all clinical incidents, complaints and untoward incidents in relation to looked after children, care leavers and unaccompanied asylum seeking children.
Contribute to the development of protocols, guidelines and policies appropriate to the post working collaboratively with other staff and agencies.
Develop and maintain multi-agency partnerships to ensure looked after children, care leavers and unaccompanied asylum seeking children are prioritised in accessing health services.
Deputise for the Named Nurse as appropriate
EDUCATION AND DEVELOPMENT
The post holder will:
Contribute to the development of strategies, policies, procedures and guidance across health and social care and implement, review and evaluate them.
Work collaboratively with other professionals and other agencies (e.g. Aftercare Team, Medical Advisor for Fostering and Adoption, Homeless Team, School Nursing and Health Visiting Teams, Teenage Pregnancy, & Childrens Emotional and Mental Health Services; Residential Childrens Homes; Voluntary sector, Designated Professionals).
Contribute on the development of integrated approaches to service development and delivery in relation to Looked After Children.
Recognise opportunities and innovations within practice, influencing, sharing and disseminating good practice at a local and regional level.
Improve service development and improve outcomes for Looked after Children in line with statutory guidance.
Share own skills, knowledge and expertise in relation to looked after children, care leavers and unaccompanied asylum seeking children, supporting others in their developments.
Effectively engage with service users in relation to looked after children, care leavers and unaccompanied asylum seeking children to meet the outcomes of this vulnerable group.
Liaise with other Looked after Childrens Teams nationally to ensure effective working relationships and promote positive outcomes for Looked after Children.
RESEARCH AND AUDIT
The post holder will:
Proactively provide easily accessible information in a form, which meets the understanding of all young people and their carers.
Disseminate information in an appropriate and meaningful manner across health and social care.
Assist with the accurate and timely collection of health data and analyse it using appropriate tools and software.
