Job summary

The post holder, as part of a team, will provide a flexible service to respond to the physical health care needs of patients within Ashworth Hospital. In conjunction with the multidisciplinary team and external service providers. The post holder will be an autonomous practitioner demonstrating skills and competency in the assessing and treating patients with a variety of illnesses/ Chronic diseases.The post holder will ensure that all relevant patients are placed on an appropriate chronic disease and are assessed and monitored in line with current guidelines.The role of the nurse in High Secure Service (Ashworth Hospital) is to treat service users, with known dangerous, criminal, or violent propensities and can involve exposure to highly distressing and emotional circumstances.The post holder must plan, evaluate and implement care using both nursing and medical models to ensure appropriate assessment and treatment of patients.

Job responsibilities

To participate in clinical audit and research activities as appropriate to maintain and improve professional development and the quality and efficiency of the service according to changing patient need which directly contributes to long term strategic planning.2. To use specialist skills and knowledge to advise and inform the multi-disciplinary team in the ongoing management of chronic disease problems. To increase nursing competency through education of ward staff for care of individual nursing needs.3. To receive and communicate highly complex information to patients relating to chronic disease where there are barriers to understanding due to mental health issues.4. To maintain up to date core skills and extend professional role in conjunction with strategic development of PHC services.5. To promote and encourage evidence-based practice with the multi-disciplinary teams in line with National Service Frameworks and NICE guidelines. Intense concentration is required to study and retain information to ensure that evidence-based practice is maintained.6. To make judgments on clinical/security issues, ensuring patient and staff safety through appropriate risk assessments.7. The post holder is required to take an active role in goal planning of service development in the forthcoming years.8. To provide advice and support to all members of the multi disciplinary team to understand their role in the promotion of their patients health in relation to chronic disease.9. To develop effective communications across high secure services to ensure awareness of issues affecting the provision of effective health promotion and chronic disease management.10. To undertake full control and restraint training, manage situations of actual and potential C&R issues using either approved C&R techniques or verbal de-escalation skills.11. To ensure regular risk assessments are carried out for all activities undertaken.12. To ensure that safety is paramount for staff and patients taking the required actions to avoid unnecessary risks, and to report any concerns and/or deficits to the Modern Matron13. To carry out screening tests utilising laboratory type equipment and handling blood and body fluids.14. To review detailed documents in the summarising of medical records.15. To organise, plan and manage nurse led clinics for chronic disease management monitoring with the ability to prioritise workload according to physical need to improve quality and ensure service development.16. To be responsible for developing and organizing chronic disease management programs i.e., asthma, diabetes, cardiovascular diseases and assess risk factors leading to ill health and advise and assist in suitable interventions.17. To assist in the development of protocols and guidelines for use with patients in the health Centre, liaising with GPs, Primary Care Trusts, and national agencies.18. To assist with devising and implementing policies relevant to physical health care.19. To be responsible for accurate documentation and record keeping of clinical findings, having an awareness of alternative method of data collection for patients physical health care records for data collection purposes, i.e., PACIS/VISION.20. To maintain responsibility for Health & Safety aspects of the environment for self, service users, colleagues and visitors.21. To ensure that work is conducted in accordance with the Trusts equality and diversity strategy and to contribute to its development.22.   • Educated to degree level, hold a specialist qualification or have equivalent clinical / management experience
  • Qualified independent nurse prescriber (V300) or be willing to undertake training and take on the role of an independent prescriber
