Job summary
An exciting opportunity has arisen within the Sefton Equipment Advisory Service. The post available is for an assistant practitioner or a suitable candidate with patient facing experience.
This is a developing and evolving role that involves education, training, supporting prescribers in the issue of community equipment. As a successful candidate you would be expected to have excellent problem solving skills, interpersonal skills and a desire to further your learning and knowledge of community equipment and patient assessments.
Main duties of the job
To give advice on appropriate community equipment within the patient home environment.
To Support the prescribing of equipment in the community, to prevent hospital admissions and promote safe discharges from secondary care.
To assist the Clinical lead with training and education on safe and appropriate prescribing of equipment in the community setting.
To manage own caseload and work autonomously.
Provide clinical support and knowledge to support staff
Undertake risk assessments on defined patient in line with relevant guidelines
