Job summary

Liverpool Community Bladder and Bowel Service aim to provide care for patients with Bladder and or Bowel dysfunction.

Our specialist nurses will complete full holistic assessments to assess and promote the wellbeing of patients under our care. By following National guidelines and providing evidence based treatment we aim to promote and support patients with continence dysfunction to live independently in the community as much as possible.

You will work as part of a very experienced specialist team and will be supported to increase your knowledge and skills in bladder and bowel dysfunction.

Main duties of the job

You will undertake responsibility for a delegated caseload under the supervision of specialist nurse caseload holder.

Carrying out initial assessments and plan, review and evaluate care of the individual patient pathway.

Be involved in complex assessments as required.

Maintain and develop your clinical competencies as required to meet the needs of patients on the caseload.

Promote independence, self management of needs and provide health education/healthy lifestyles to our community

Job description

Job responsibilities

1. Provide information and advice on prescribed or over the counter medication.

2. Recognise, assess and refer patients presenting with mental health needs.

3. Promote and deliver evidenced based care.

4. Assist senior practitioners as required.

5. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

6. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural backgrounds and preferred ways of communicating.

7. Act as an advocate when representing patients and colleagues viewpoints to others.

8. Recognise and work within own competence and professional code of conduct as regulated by the relevant professional body.

9. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

10. Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

11. Deliver care according to national and local guidelines, working in partnership with other clinical teams/departments.

12. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.

13. Participate in the maintenance of quality governance systems and processes across the organisation and its activities.

14. Utilise the audit cycle as a means of evaluating quality implementing improvements where required.

15. Evaluate the patients response to health care provision and the effectiveness of care.

16. Support and participate in shared learning across the wider organisation.

17. Utilising a structured framework (e.g. root cause analysis) participate in the management, review and identify learning from patient complaints, clinical incidents and near miss events.
