Job summary

Mersey Care Foundation Trust are delivering an award winning and dynamic multidisciplinary community service across Southport & Formby known as ICRAS (Integrated Community Reablement Assessment Service).

ICRAS is focused on delivering the best of care and reducing the risk to adults including frail elderly and those who may be in a health or social care crisis, improving quality of care in line with current evidence.

ICRAS also provides bed based services within local residential and nursing homes, domiciliary care and rapid response.

This allows great opportunity to rotate around the varying aspects of the service.

Main duties of the job

The post holder will be responsible for assisting in the delivery of rehabilitation within the ICRAS/Frailty Service, providing a programme of therapeutic activities and interventions under the direction and supervision of a qualified Therapist.

The post holder will be required to actively work as part of the multi-disciplinary team, supporting short term crisis care when required.

The post holder will be expected to work in an innovative way to support the smooth running of the Service and will be required to offer individual and group activities alongside other therapy staff and under the indirect and direct supervision of a Qualified Therapist.

  • NVQ Level 2, with an aim to complete NVQ Level 3 at a later date

Desirable

  • Other qualifications linked to role, ie, gym instructor qualification

Experience

  • Experience of working in the NHS, community care, or rehabilitation environment

  • Experience of working with people who are frail, elderly, or have long term conditions.
  • Experience in handling sensitive situations
  • Creativity and ability to adapt activities to the needs of the patients
