Job summary

The Urgent Community Response Service in Knowsley provides care and support for patients over the age of 18 years old and whom are at risk of hospital admission. The Urgent Community Response team provide a two hour crisis response to a range of nursing interventions. The service is delivered jointly by Mersey Care and Knowsley Borough Council to provide urgent health and social care to patients.

In addition, the service is in the process of expanding its clinical workforce to support the implementation of the Virtual Wards by the Acute Hospital Trusts. The Urgent Community Response team will support those patients admitted to the Virtual wards to remain in the community.

We are seeking to appoint a highly enthusiastic and motivated individual to join the service as a Health Care Assistant.

Successful applicant would work under the direction of the Urgent Community Response Team leaders supporting the provision of high quality holistic nursing care to patients. The Health Care Assistant will provide care to patients in their own homes and other community settings, following appropriate training and supervision

Operational hours of the service are 8am-10pm 7 days per week 365 days per year. Successful candidates would need to support the service between the hours 8am-8pm 7 days per week and will also need to be flexible to meet the demands of the service.

Main duties of the job

The post holder will work under supervision as part of the multi-disciplinary team and will assist and support the clinical team(s). This may include working within the patient's home environment and other clinical settings in the community.

Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
