Job summary

An exciting opportunity has arisen within the District Nursing Service. We are seeking to appoint a highly enthusiastic and motivated individual to join the service as a Health Care Assistant. Successful applicant would work under the direction of the district nursing Team leader supporting the provision of high quality holistic nursing care to patients. The Health Care Assistant will work without direct supervision on occasions providing care to patients in their own homes and other community settings, as delegated by the District Nursing Team leader following appropriate training and supervision to ensure clinical competence and safe effective care.

The service operates from 8am-8pm 7 days a week

Main duties of the job

To work as part of the Integrated Neighbourhood/ Locality team under the direction of the district nursing Team leader, supporting the provision of high quality holistic nursing care to patients.

The Health Care Assistant will sometimes work without direct supervision providing care to patients in their own homes and other community settings, as delegated by the District Nursing Team Leader following appropriate training and supervision to ensure clinical competence and safe effective care, but will also work with the trained nurse providing substantial input in palliative care

To support other health care professionals in the provision of high quality care in the locality population.

Job description

Job responsibilities

1. Work as a member of the multi-disciplinary team.

2. Undertake a range of patient interventions within the scope of NVQ level 3 skills for patients as delegated by team leader or other qualified clinical staff.

3. Offer advice to patients and carers on a wide range of health issues to include health promotion, lifestyle advice and self-care.

4.Plan and organise own work schedule once delegated from team leader.

5.Undertake basic patient observations.

6. Work within existing clinical guidelines, policies and infection control procedures.

7. Offer advice and support to patients and families in a wide range of situations.

8. Communicate factual and sensitive information within confidentiality guidelines to patients, ensuring all information is appropriately documented and reported back to the team leader.

9. Demonstrate own role to new starters and students.

10. Ensure safe moving and handling techniques in the delivery of patient care, this can include the safe use of equipment.

11. Communicate information to multi-disciplinary team and other agencies as appropriate.

12. Undertake ordering and receipt of stores.

13. Undertake further training to gain skills in other areas.

14. Utilise NVQ level 3 skills within identified areas as required by the clinical team.

15. Deliver patient care within essence of care standards and Trust guidelines and policies.

16. Document care delivered within hand-written or electronic patient records.

17. Participate in own PDR with team leader to establish objectives, reporting arrangements, supervision and ongoing development.

18. Participate in mandatory training events.

19. Work with community groups to improve patient participation and involvement.

* The post holder shall as necessary provide cover for and undertake duties of absent colleagues.

* The post holder shall follow all the policies and procedures of the organisation.
