Job summary
Warrington Recovery Team (Adult Community Hub) are looking for an experienced, skilled and dedicated senior nurse practitioner to work in our Clozaril Clinic.
The role will require you to facilitate the day to day delivery of our Clozaril service, providing care, treatment and support to service users in the Team, who are prescribed Clozaril medication.
You will work alongside experienced colleagues, to provide close monitoring of our service users physical and mental health. You will require a good working knowledge around physical and mental health and be confident in liaising with other professionals and agencies, in relation to the health needs of our service users.
Our operating hours are currently Mon to Fri 9-5 and our clinic base is Hollins Park, Hollins Lane Warrington.
Main duties of the job
- To provide a lead role in the Clozapine Clinic
- To work as a member of the Multi-disciplinary Team if there are any concerns or if the Service user fails to attend the clinic
- To work within the scope of the Trusts Clozapine Prescribing and Monitoring Procedure
- To have knowledge and understanding of potential side effects of Clozapine, the recognition of side effects and liaison with the MDT regarding action to address concerns and side effect
- To complete physical health monitoring of service users prescribed Clozapine and what to do if concerned about any results
- To provide information to service users and carer's regarding Clozapine Treatment
- To have knowledge of the titration procedure for Clozapine
- To ensure all service users have annual Clozapine plasma levels completed, and understand in what circumstances these must be repeated more frequently
- To support the Recovery Team with completing annual physical health checks for service users on the caseload who require physical health monitoring
- To be trained in venepuncture, ECG testing, and will be expected to carry out routine physical health observations.
- To escalate any abnormal physical health results or concerns to the most appropriate service. (They could access ECG and Venepuncture training in trust if they don't already have this)
- To have an understanding of NEWS2 Physical Health and how this is used in practice
- To be able to complete mental state assessment and act on any concerns

You will receive ongoing clinical and management supervision and work closely as part of an experienced MDT.
