Job summary
This postholder will be required to work closely within the multidisciplinary team and other professionals, particularly in relation to patient observations, mobility and enablement, including assessment for and provision of appropriate equipment.
They will provide specialist and skilled assistance in the delivery of Physiotherapy / Occupational Therapy / Rehabilitation for adults / older people and will plan, review and independently implement specialist rehabilitation and enablement programs for a variety of clients in varying settings, within the Long COVID service.
You will play a vital role in supporting the delivery of integrated, co-ordinated, comprehensive, seamless, person-centred care. As a member of the Tier 3 Long COVID Assessment & Treatment service Multidisciplinary Team (MDT) you will be required to make recommendations based on the principles of shared decision making for people living with or experiencing the impact of long covid symptoms and access Tier 3 Services.
The successful applicant will work both from clinic settings, and also in the community, covering the borough of Sefton.
Main duties of the job
As a member of the Tier 3 Long COVID Assessment & Treatment service Multidisciplinary Team (MDT) you will be required to make recommendations and will always take account of the views, preferences and circumstances of patients/service users, parents or carers when considering clinical and non-clinical decision-making. You will be required to provide specialist and skilled assistance in the delivery of Physiotherapy / Occupational Therapy / Rehabilitation for adults referred in to the Long COVID Tier 3 service and will plan, review and independently implement specialist rehabilitation and enablement programs for a variety of clients in varying settings, within a defined area of competence.
You will work closely with multidisciplinary teams and other professionals, particularly in relation to patient mobility and enablement, including assessment for and provision of appropriate equipment, and to assist in the development of good working relationships with statutory, voluntary and independent organisations.
