Job summary
The post holder will be an autonomous practitioner expected to take an active role in the delivery of advanced assessment, and the management and treatment of patients over 18 years of age patients with inflammatory bowel disease (IBD) .
The role will provide comprehensive healthcare sensitive to the needs of patients with IBD, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate. Working as part of a multidisciplinary team and across the GIMS single managed service, the post holder will be responsible for co-ordinating the delivery of urgent health care needs of people with IBD.
The post holder will provide specialised in-reach to the Emergency Department to triage, assess and input as an independent practitioner to patients with an exacerbation of their IBD. The focus will be to prevent unnecessary acute hospital admissions and enable patients to remain safely and supported at home.
The post holder will provide specialist care to inpatients with IBD, assist patients to proactively manage and anticipate their future health needs and develop appropriate plans in conjunction with the Consultant of the Week and the IBD service. It is expected that the post holder will provide IBD care in an outpatient setting to both new and already diagnosed patients.
The post holder will lead in developing, delivering and evaluating high quality patient centred interventions in conjunction with the associated clinical teams.
Main duties of the job
The main duties of the post include, but are not exhaustive:
1. Uses advanced clinical skills and judgement in managing complex and unstable IBD. As an autonomous practitioner assesses, plans, implements and evaluates care for patients.
2. Recognise the early symptoms of disease exacerbation and acute illness based on an understanding of disease, the disease process, and current evidence and practice standards.
3. Assessment of IBD (including unstable or complex disease) through clinical history taking and onward referral to endoscopy, radiology, surgery and other appropriate specialties
4. Assessment of perianal Crohn's disease, including:- requesting of MR pelvis - requesting endoscopy - referring to surgical teams for assessment
Autonomously develops treatment plans and escalates treatment to prevent complications developing. This may include referral for escalation to biologic therapy to the IBD MDT.
