Job summary

Are you an experienced Nursing Assistant/ senior support worker that is passionate about keeping patients in the comfort of their own home to avoid hospital admissions, or supporting patients discharged home after a lengthy hospital stay? Then this is the role for you.

We are a service in North Manchester aiming to reduce the need for people to go into hospital by wrapping expert care around them in the community.

As part of the Discharge to Assess Team you will be part of a wider multidisciplinary team which includes Occupational Therapists, Physiotherapists, Nurses and Pharmacy. You may also have the opportunity to work with wider community urgent care services such as; Crisis response, hospital at home and the clinical navigators.

We will support you with specialist and bespoke training to reach your potential and excel in your chosen career path. You will receive one to one clinical support, professional development, and guidance.

This role is a seven day service working between the hours of 8am-9.30 pm on a rota basis. We promote flexibility and are happy to support different working patterns within the needs of the service.

Main duties of the job

In the role as Senior Nursing Assistant, working without direct supervision you will be responsible for delivering clinical care for patients in their own home. With Immediate access to a qualified staff member for advice and direction on care as required.

You will be responsible for your prioritising your own caseload of work and be responsible for tasks delegated from a registered nurse.

Communicating effectively with patients, relatives, carers, and other healthcare practitioners, ensuring any concerns are listened to and relevant actions are taken.

You will always act in a manner, which illustrates respect for privacy, dignity and confidentiality.

A full UK driving licence with access to a vehicle is required for this role as you will be Travelling across the North Manchester Community locality.
