We are looking for a motivated Physiotherapist with relevant skills and experience to join the Discharge to Assess Trafford Team. An opportunity to work in a supportive team, supporting a busy, productive service as part of the Trafford Community Response Teams.
Full time over 7 days to include weekends and bank holidays

Our teams work as one in the neighbourhoods they serve - delivering and designing services in partnership with local people.
The Discharge to Assess service will be a seven day service, therefore this entails working some weekends, bank holidays and late shifts on a rota basis. The post involves working across the main hospital site and the community, delivering holistic assessments & interventions to patients in their own homes/ place of residence.
Independent travel within the local geographical area to deliver the role is essential.

Main duties of the job
The post-holder will triage, assess, implement, plan and evaluate the delivery of direct patient care as an independent practitioner relevant to their Band.
Working as a practitioner within a multi-disciplinary / multi-agency service, they will facilitate timely discharge from an acute hospital or community bed and provide same day assessment and support planning to meet a person's needs in the community.
The post holder will provide and / or arrange appropriate support and interventions in the community working closely with colleagues across all sectors. They will work flexibly providing therapy support in the community as required to provide the full discharge to assess service cover.
Working as an integral part of the team your role will be to provide specialist assessment skills, knowledge and management of patients in the community whilst ensuring the delivery of high caliber care, and efficient use of facilities, manpower and other resources.
The post holder will provide specialist knowledge and clinical skills to ensure the delivery of care within the D2A team and wider TCR service as required. Working to ensure the patients can be managed safely at home following a hospital admission.
