Job summary

Community Respiratory Team based at Wythenshawe Hospital. Community work is across South Manchester area (typically Wythenshawe, Northenden, Withington, Didsbury)

Service roles include

In patient reviews of COPD and asthma patients

Home visits to assess COPD patients

Assessments and running pulmonary rehabilitation classes

In-patient Oxygen assessment and ordering

7 days service 8am-6pm - flexible hours to meet these times (typical shift pattern involves a weekend day approximately 1 in 5, please call and enquire if any further details needed)

Main duties of the job

As a specialist nurse/physiotherpaist , you will play a central role in delivering and shaping high-quality respiratory care across South Manchester. Our service is consultant-led, ensuring strong clinical governance and direct access to specialist expertise. Based at Wythenshawe Hospital -- a nationally recognised centre of excellence for respiratory care.

Your responsibilities will include:

  • Contributing to a multidisciplinary team of nurses and physiotherapists across community and hospital settings
  • Providing specialist assessment and treatment planning on acute home visits, pulmonary rehabilitation, COPD/asthma care bundles, and oxygen assessments
  • Managing and prioritising chnaging acute caseloads, including patients with COPD in the community.
  • Working autonomously across community locations, supported by senior clinical leads and a respiratory consultant
  • Ensuring safe, evidence-based practice and contributing to continuous quality improvement
  • Supporting the education, supervision, and development of students
  • Championing an inclusive, collaborative working environment where diverse perspectives strengthen patient care and team performance
  • Deliver patient education in 1:1 settings as well as group patient education regularly

  • Relevant professional qualification BSc (Hons) Physiotherapy or equivalent / professional qualification RN/RGN
  • Active HPC registration/NMC pin
  • CPD Portfolio

Desirable

  • Related advanced respiratory qualification

Experience

Essential

  • Experience of working within a respiratory care setting especially chronic lung disease, minimum of 6 months
  • Relevant experience at band 5 level
  • Experience of working as part of an MDT
  • Evidence of delivering teaching and education to staff groups and patients

  • Experience of community working and PCT partnership working
  • Experience of service development and evaluation of service standards through clinical audit and research.
