Job summary
We are seeking motivated and compassionatePathway Navigators to join our specialist Interstitial Lung Disease (ILD) team. This is an exciting opportunity to play a pivotal role in supporting patients with complex respiratory conditions, ensuring they receive timely, coordinated, and person-centred care throughout their treatment journey.
As a Pathway Navigator, you will act as the first point of contact for patients and their families, guiding them through diagnostic, treatment, and follow-up pathways. You will work closely with clinicians, nurses, allied health professionals, and administrative staff to streamline processes and improve patient experience.
There are 2 posts available with 0.6 WTE based at North Manchester General Hospital and 0.6 WTE based at Manchester Royal Infirmary Hospital
Main duties of the job

Deliver a high-quality administrative and secretarial service to the ILD team.

Coordinate weekly MDT meetings, including listing newly diagnosed patients and recording treatment decisions.

Book and manage outpatient appointments, ensuring smooth patient flow through the pathway.

Track patients against national targets and NICE recommendations, escalating delays to prevent RTT breaches.

Collect, record, and report patient data for audit and research purposes, including real-time reporting to external organisations.

Coordinate diagnostic activity (radiology, bloods, physiology), reducing duplication and ensuring right patient, right test, right time.

Manage the ILD waiting list in line with the Trust's cross-site pathway.

Act as the first point of contact for complex enquiries, handling sensitive information with professionalism.
