Band 6 Rotational Physiotherapist
We are excited to be advertising for a band 6 Specialist Physiotherapist rotation between Wythenshawe Hospital's Older Persons Assessment and Liaison (OPAL) and Integrated Assessment Team (IAT), and South Manchester's Community Response Services of Discharge to Assess (D2A) and Crisis response.
We are advertising for 1 permanent, full time position, and are looking for someone with exceptional generic assessment and therapeutic skills, who is motivated to deliver care of the highest quality through personal and professional development and collaborative multi-disciplinary working. Applicants are required to hold a UK driving licence and have access to their own vehicle for business purposes.
There are three rotations:
Wythenshawe Hospital's OPAL/IAT
South Manchester D2A Team
South Manchester Crisis Response Team
Main duties of the job
OPAL/IAT deliver high quality care to our patients across the Emergency Department, Acute Frailty Unit and the Acute Medical Unit. The aim of our service is to improve outcomes for our older population, and those living with frailty through proactive early assessment and management of complex co-morbidities, preventing unnecessary hospital admissions, supporting early discharge and accessing appropriate community services. This rotation requires respiratory ward level competency.
The Crisis Response Team support people with a health or social care crisis to stay at home and avoid unnecessary hospital admission. We provide same-day assessment by our integrated, multi-disciplinary team to make safe and stabilise. We can support at home for 48 hours.
The Discharge to Assess Team supports timely assessment of patients at home at the point of discharge to identify health and social care needs that can be delivered in the community for up to 7 days. The Discharge to Assess Team works from 8am until 8pm 7 days per week.
Applicants should be highly motivated, enthusiastic and experienced clinician
You will consolidate knowledge and develop clinical and non-clinical skills within a supportive environment and be actively involved in the development of the service, supervision of junior staff and student education.
