Job overview

The post involves working within the discharge to assess team delivering holistic assessments and interventions to patients in their own homes/ place of residence. Independent travel within the local geographical area to deliver the role is essential.

The Manchester community response services offer a seven-day service:

Discharge to Assess- delivered over 7 days: Mon- Fri 08:00-20:00 / Sat - Sun: 0800- 1600

With the above in mind, you must have experience of working in the NHS and in community services. The successful candidate will be able to demonstrate an appropriate level of clinical skills in physiotherapy and will be experienced in administering person-centred assessments. They will be able to implement and evaluate physiotherapy interventions in a fast paced working environment. You must have a recognised Physiotherapy qualification and current HCPC registration.

Main duties of the job

The individual will manage a clinical caseload of patients, including those with complex and highly complex needs. You will supervise the band 3 support staff with caseload management.

The post holder will work without direct supervision as a lone worker in the community assessing, planning, implementing, evaluating and modifying individualised treatment programmes in patients' homes for patients with low to highly complex needs.

The post holder will work as a team member collaborating with colleagues to ensure patient needs and goals are met using approaches which promote choice, participation and quality of life.

The post holder will ensure patients are triaged and managed on appropriate and timely pathways of care by liaising closely and working in partnership with other community therapy teams as well as health and social care professionals including GPs, specialist equipment agencies, social service partners and voluntary agencies. You will support patient flow through supporting the timely discharge of patients from the acute setting.

The post holder will provide complex professional and clinical advice to patients, family members, carers and health and social care professionals including GPs, therapy colleagues and care managers.
