An exciting opportunity has arisen to be part of a new project delivering SACT in patient's homes and on a mobile cancer care unit, a 12-month pilot for patients undergoing treatment for haematology/Oncology conditions.
The band 6 nurse will be responsible for assessing, developing, implementing and evaluated family centred care in line with the project. Phase 1 of the pilot will be delivering SACT treatments in patient's homes and phase 2 will be administering SACT on a Mobile cancer care unit.
You must have excellent communication skills and work well as part of a team, able to recognise limitations in line with using initiative.
The post holder will need to have access to a car and hold a full driver's license.
Main duties of the job
The post holder will be responsible for a caseload and be responsible for the assessment of care needs, plan implement and evaluate the care for patients in the care closer to home programme.
The post holder will provide direct patient care and have detailed clinical knowledge of the specialist area. The post holder will ensure that high standards of quality of care are delivered & maintained. The post holder will act as a positive role model within the department and in the community for staff patients and carers.
They will effectively contribute to the care closer to home project and ensure the provision meets the needs of the patients and families.
The post holder will enable nurses to acquire clinical and professional competencies by facilitating high quality education and staff development opportunities
