Patient Pathway Co-Ordinator

Job summary
The post holder will be the point of contact for all administrative issues relating to patients' pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, focused, and knowledgeable point of contact. To provide a consistent approach across the Trust, Standard Operating Procedures (SOPs) and policies will describe how the functions of the role will be carried out, along with the timescales to which are to be adhered.
The post holder may be required to work across services within the directorate and across WTWA sites
Main duties of the job
To ensure clinic cancellation requests are submitted following approval of clinicians leave and monitor Health Roster to ensure the clinics are cancelled / moved as per instruction. Monitor any changes to clinics to ensure that patients do not breach timeframes and standards.
Support monitoring of clinic utilisation compliance with the Service Manager.
Work proactively and closely with the outpatient waiting list manager to ensure full utilisation of clinics and early intervention when an issue is identified, escalating to Service Manager where appropriate.
Supports the administrative process for any applicable Multidisciplinary Team Meetings, Governance processes and RCA's to ensure all necessary and appropriate clinical information is available for discussion at set meetings.
Work closely with medical, nursing, advanced healthcare professionals and Operational Management team.
