Cardiac Surgery Senior Diabetes Nurse Specialist

30 hours Permanent Position

An exciting opportunity has arisen for an experienced Clinical Nurse Diabetic Specialist to work with cardiac surgery patients, providing expertise in diabetes care by providing information, support advice and specialist knowledge to patients, their families and carers, and act as a resource to the multi-professional team.

Main duties of the job

To participate in the co-ordination of patient care to ensure continuity throughout the patient's care pathway. To contribute to review and development of the service, and to reflect the needs of the patient group. To review HBA1C in all new patients to the cardiac surgery waiting list and non-elective list and optimise management of existing patients.

To liaise closely with the health care teams of inpatient wards, outpatient department and primary care, including Consultant Physicians and GPs as necessary. To demonstrate knowledge and experience of diabetes management and clinical leadership skills. To monitor and improve standards of care through clinical audit.

Be willing to undertake further training and development as required to achieve the requirements of the post. To support the delivery of a high quality, safe and compassionate healthcare service, all staff are expected to act as a role model to others in all aspects of their work and consistently demonstrate MFT values.
