Job summary

An exciting opportunity has arisen to join Central Manchester Crisis Response Team.

The post holder will be responsible for providing a rapid assessment and stabilisation of patients who have urgent care needs in the community and initiate services to safely avoid unnecessary hospitalisation. This will be through assessment and treatment of patients during a crisis, identifying complex and specialist needs and developing, implement and evaluating programmes of care.

Main duties of the job

Perform comprehensive assessment of patients with diverse physical, social & medical presentations to provide a clinical assessment and evidence based intervention and effective discharge management.

Be professionally and legally accountable for all aspects of own work, including the management of patients in your care, and that of junior staff and students within the service.

Undertake comprehensive assessments of patients with diverse or complex medical presentations and multi-pathologies using analysis, clinical reasoning skills, manual handling techniques and listening skills to receive complex & sensitive details from patients to provide an accurate diagnosis of their condition.

To use confident decision - making skills to contribute to the multi-disciplinary decision making process to rapidly determine suitable discharge destination or to determine an appropriate place ofadmission if admission avoidance is not possible. Good awareness of limitations of own knowledge and skills and when to escalate to senior clinicians.
