Diabetes Specialist Nurse

Job summary

An exciting opportunity has arisen for an existing band 6 Adult nurse to join our small, dynamic community specialist diabetes team in a full-time band 6 development role on a clinical pathway to development to a band 7 Diabetes Specialist Nurse.

The successful candidate will be provided with the specialist training and support required to become a Diabetes Specialist Nurse (DSN).

You must be a forward thinking and dynamic individual with a passion for diabetes management and a strong interest and commitment to support the service in driving forward national diabetes care agenda and deliver high quality, effective and safe patient care.

Main duties of the job

To work as an advanced autonomous practitioner, to be professionally and legally accountable and responsible for all aspects of patient's diabetes care including decision making around the treatment and management of patients with complex physical and social needs

To maintain accurate and comprehensive documentation

To hold a caseload of patients with a range of complex needs requiring advanced intervention to improve patient outcomes.

To prescribe appropriate medication as independent non medical prescriber V300, to maintain and keep up to date with prescribing practices using evidence based care

To work as a specialist practitioner supporting patients within both a clinic setting and working closely with community nursing teams to manage complex patients cared for within their own homes.

To communicate complex and sensitive information and education to a wide range of patients requiring excellent interpersonal and communication skills

To provide Type 2 diabetes education within the community setting

To provide a resource for MDT members providing advice and guidance. To provide mentorship to student nurses, new staff members and primary care staff as required
