Job summary
An exciting and unique opportunity has arisen for a Band 6 Diabetes Specialist Nurse to join the Manchester Adult Cystic Fibrosis Centre's multidisciplinary team.
We are looking for a self-motivated, dynamic individual to join the established dedicated diabetes service. You will be working alongside a multi-professional team of CF and diabetes specialist delivering education, care and support to people with Cystic Fibrosis Related Diabetes within a variety of settings.
The successful candidate will work 30 hours over a 4 day period as part of a large MDT across primary and secondary settings with the aim of:
Providing and supporting high quality holistic, individualised care to patients with complex diabetes management needs.
Promoting a greater understanding of diabetes and empowering patients, their families and carers and other agencies and professionals involved with the patient through education, support and counselling.
Alongside a senior diabetes specialist nurse you will be responsible for supporting a caseload of patients with complex diabetes management needs, using evidence based practice and patient centred goals to assess, plan, implement and evaluate care and interventions.
MACFC is a regional unit therefore the successful candidate would need to be prepared to travel within the North West region.
Main duties of the job
To act as an autonomous practitioner with freedom to initiate action within broad clinical / professional guidelines.
To communicate closely with the Multi-disciplinary team to ensure that patients receive optimum care by regular attendance.
To work with the team in the stabilization of patients on insulin therapy within agreed protocols and guidelines.
To provide holistic, high quality, evidence based nursing care to patients with CFRD in a variety of hospital and community settings.
To accept clinical responsibility for the designated caseload of diabetes patients both inpatients and outpatients and to organize this effectively with regard to clinical priorities and use of time.
To participate in the development of specialist diabetes care such as Insulin Pump Therapy and Continuous Glucose Monitoring System (CGMS).
To provide counselling, advice and support to patients and carers in the complex treatment regimes in order to maximize adherence to diabetes and CF therapies.
Organize Diabetic Annual reviews with feedback of results to MDT and GP
To liaise with GP/Practice Nurse to facilitate the referral of patients into retinal and foot screening programmes in the Community.
To maintain accurate and up to date diabetes records that meets the Trust and NMC standards.
