You will positively contribute to patients remaining in their own homes and out of hospital, by working in a multidisciplinary team.

The community-based team works closely with the Manchester Foundation Trust (MFT) Pharmacy, a large forward-thinking department, enabling access to training programmes and excellent career-development opportunities.
You must have a substantial amount of hospital or primary care experience, be a Pharmacy technician registered with the General Pharmaceutical Council (GPhC). You must have access to a car for work purposes.
Main duties of the job
The North MCR pharmacy technician is based at Harpurhey office working with the MCR pharmacists providing medicines optimisation support to the Discharge To Asses and CRISIS services. The technician will contact the patients and families/carers to discuss their medicines and carry out domiciliary visits as required.
The role includes:
- Working in the multidisciplinary team to:
- Provide better co-ordinated care to patients
- Support safe discharge from hospital
- Reduce hospital re-admissions
- Improve medicines safety
- Improve medicines compliance
- Liaise with pharmacists, technicians and other health care professionals in secondary and primary care to ensure continuity of care for patients
- Carry out medicines review with patients
- Identify and act on medication errors/queries, reporting complex issues to a pharmacist as appropriate.
- Contribute to new service developments and projects
- Provide support to other CMOS workstreams as necessary.
- Participate in any arrangements that may involve flexible working, e.g. extended hours or 7-day service.
