Job summary

An opportunity has arisen for a Patient Flow Coordinator on ward A6.

Ward A6 is a 28 bedded surgical receiving ward taking direct admissions from accident and emergency, clinics and SACRU. Ward A6 provides care to mixed surgical specialty, including general surgery, urology, orthopedics and ENT. Good discharge planning starts on admission and is essential for ensuring a smooth transition of care back into the community.

As a patient flow assistant you will be required to assess a patients usual/pre admission support in the community and to liaise with the multidisciplinary team during the patients admission to ensure the right support is requested and confirmed for discharge.

Main duties of the job

A6 is a Mixed Specialty Surgical receiving unit caring for a wide variety of patients. The multi-professional team (nursing, medical and therapy teams) work closely together to provide a patient-centered and coordinated service which supports the timely discharge of patients following surgery, as well as ensuring that MFT meets the 4 hour target for our patients who are admitted as an emergency through the Emergency Department.

The successful candidate will be able to demonstrate that they are compassionate, possess excellent team working and communication skills and be flexible to meet the needs of the ward, working in a new dynamic role supporting the ward with patient flow and assisting the ward manager.

The role of Patient Flow Coordinator is a busy and varied one where the work will change to meet the needs of the service. This role includes clinical patient contact, and you will be integral to supporting the admission and discharge process and support a team of multi-disciplinary clinicians. You will regularly liaise with multi-agency services, including Social Services and the private sector to ensure our patients safety is kept paramount beyond discharge from our care.
