Job summary

We are looking for an experienced and motivated Band 6 physiotherapist who will work with our occupational therapist and nurse team leads, sharing our passion for supporting older people living with frailty and our enthusiasm for making a difference.

Hospital at Home will enable patients to receive safe, treatment in their own homes. This is a truly exciting time to join us as we forge strong links/relationships across our hospital and community to further enhance this model of care and support our older people to receive care in the comfort of their own home through Hospital at Home service.

We are an enthusiastic, motivated multi-disciplinary team working in a dynamic, exciting environment, providing an assessment, discharge, and hospital avoidance service. Embracing these models, we will support the NMGH Emergency department, AMU and Clinical decision unit and work closely with our frailty team both at the front door of the hospital providing specialist discharge planning support, assessment, and advice to ward areas.

Main duties of the job

The post holder will work as part of the Hospital at Home team to co-ordinate the discharge of selected patients and to be involved in the rapid response function of the service. The post holder will be working as part of a multidisciplinary and multiagency team and in collaboration with other services including the acute and community- teams to facilitate the timely assessment and appropriate discharge from hospital through assessment, care planning and integrated working responsive to patient needs.

The postholder will provide and/or arrange appropriate support and interventions in the community working closely with colleagues across all sectors. The post holder will provide specialist knowledge and clinical skills to ensure the delivery of care within the service and also the wider community and across the acute / community interface. They have a fundamental role in the delivery of high standards of care, ensuring an environment that is conducive to well-being. They will actively participate in improving the service the department/unit offers and will initiate and support future developments.
