Job summary

We have a new and exciting opportunity within the Specialist Respiratory team for a MDT Co-ordinator, band 4 within the Post Acute COVID Assessment Service (PACAS).

This role is part of an NHS England incentive to provide specialist multidisciplinary support to patients experiencing long term health effects following COVID-19 infection.

The successful candidate will include providing administrative support to the Clinical team and ensuring all MDT preparation is completed in advance of the weekly MDT meetings.

All patients pathways will need to be monitored and the post holder will need to ensure any actions from the MDT are actioned. It is also an opportunity to support gathering information on patient experience via friends and family feedback and provide data to support compliance with this national programme.

The successful candidate will need to have strong organisational and effective time management skills.

Main duties of the job

  1. To have an understanding of patients' individual needs and personalised plans of care for Post Acute COVID patients following review from the clinical teams ensuring multi-disciplinary, patient centred care.
  2. Ensure PACAS MDTS run smoothly and all papers pre and post MDT are complete
  3. Ensure appointments are coordinated according to MDT recommendations, minimising the number of patients hospitals visits as appropriate.
  4. Monitor investigations and tests to ensure they have been completed and results/reports are available prior to appointments and consultations and MDT meetings with the clinical team.
  5. Communicate with the clinical team to monitor the progress of all patients according to the Post Acute COVID Service Pathways in line with local and national standards whilst identifying and minimising any delays.
  6. Promote effective inter departmental relationships with departments to ensure the Post Acute COVID patient journey is efficient and effective.
  7. Support the work of colleagues in the Multidisciplinary Team.
  8. Work with clinical and non-clinical colleagues to identify and develop administrative processes within the Post Acute COVID Service in Specialist Respiratory.
  9. Monitor and review patient waiting lists, ensuring patients are booked into the appropriate outpatient clinical setting as per Standard Operating Procedure.
