Job summary
We are looking for a qualified Occupational Therapist to join our supportive and dynamic intermediate care Team. Crumpsall vale is part of our Manchester Community Response Service (MCR) which also includes Discharge to Assess,Crisis Team, Reablement, Community flow and Community IV team.
This is an exciting time to be part of our integrated multi disciplinary and multi- agency team providing specialist assessment and interventions to our local population.
IMC aims to reduce the need for people to go into hospital or prepare them to go back to community safely by providing specialist assessment and intervention in a bed based unit.
The post holder will work in the bed based unit Crumpsall vale to provide a complex/ non complex assessment of patients who have care needs in the community and need rehabilitation before returning to home. Post holder will work collaboratively with other services to avoid unnecessary hospitalization.
Main duties of the job
The post holder will provide highly specialist knowledge and clinical skills to ensure the delivery of care within the service and also in the wider community and across the acute / community interface. They have a fundamental role in the delivery of high standards of care, ensuring an environment that is conducive to well-being.
To perform initial and profession specific specialist comprehensive assessment of patients with diverse physical, social & medical presentations to provide a clinical assessment and evidence based intervention and effective discharge management.
The role also involves;
Assessments and interventions for a designated case load on daily basis
Hold responsibility for complex assessment and management and be aware of collective responsibility for the team's caseload, working without direct supervision. Clinical work and documentation will be routinely evaluated in line with Trust policy.
