Job summary

An exciting opportunity has arisen for a qualified and experienced physiotherapist to join the Hospital and Community Team. Within the team you will have opportunity to build and enhance your rehabilitation skills in a community setting working within a friendly and supportive team.

We are a well established team of physiotherapists, occupational therapists and rehabilitation assistants based currently at Gorton Hub office.

The team provides a supported hospital discharge and ongoing community rehabilitation service to patients across central Manchester, aiming to assist them to reach their desired potential in their home environment. You will work with patients who have complex needs, providing community rehabilitation after their discharge from hospital. There may also in the future be potential opportunities to work with other teams within Manchester Local Care Organisation.

You will benefit from working in our multi professional AHP team where there is a strong culture of shared learning and support available to ensure high quality patient care. There are senior physiotherapists available to support you and your learning objectives will be agreed as part of the established appraisal process.

Continued professional development is actively encouraged. We are committed to undergraduate student education.

UK driving license and access to own vehicle are essential for the post.

Main duties of the job

Assessment, treatment and management of adult patients with wide variety of conditions, using clinical reasoning and a variety of evidence-based skills.

Working in a variety of settings including patients own homes, residential and nursing homes.

To be responsible for your own clinical caseload completing initial assessments, appropriate goal setting and interventions and interventions to address patients' rehabilitation needs

To work as part of the wider Community Rehabilitation Services to meet the demands of the services in other areas.

To support the Team Lead in the development of the service through keeping abreast of evolving research and evidence-based practice

To complete initial assessments, appropriate goal setting and interventions to address patients' rehabilitation needs.

To supervise a Band 5 physiotherapist.

Good IT, written and verbal communication skills are important as the post holder will be required to liaise with a wide range of health care professionals, patients and carers. Applicants will need to be flexible, able to work alone and as part of a team

The individual will take responsibility for the education of students.

Due to the nature of the role it is essential that applicants can drive and have access to a car for business purposes
