Job summary
The aim of this HCV pathway tracker role is to work as part of the Greater Manchester and East Cheshire Operational Delivery Network (ODN) as a Hepatitis C (HCV) results tracker and provide a robust system to ensure the safety, oversight and management of patients having HCV testing at ODN sites.
Main duties of the job
- To monitor and maintain a robust pathway for patients tested within Emergency Departments (ED) for HCV.
- To be responsible for the cohesive working between ED, internal /external services.
- Ensuring a positive result led to the patient being engaged in care at the place of patient's choice.
- Monitor incoming HCV referrals and ensure patients are linked into treatment within CQUIN timescale
- Maintain the ODN database for those patients currently failing to engage in HCV treatment.
- Support the work being undertaken by the ODN to validate the Arden & Gem HCV registry for accuracy.
- To effectively work across a range of IT systems ensuring timely results management and robust data collection.
- To devise end to end audits and report findings to clinical leads/ managers.
- Extract clinical audit data from the clinical records using internal and nationally set datasets.
- Assist clinical staff in the collection, analysis and presentation of Clinical Audit data as defined by the Unit Clinical Audit Programme and CQUIN requirements.
- Travel to off-site venues, as required to attend courses, conferences as designated by the ODN Admin Manager.
Undertake other duties as required commensurate with the post.
