We are Trafford Local Care Organisation - an exciting, public sector organisation bringing together NHS community health services and adult social care services in Trafford. We're driven by the aim of improving the health of local people in Trafford by working across organisational boundaries.
Our community healthcare teams in adults and children's services are employed by Manchester University NHS Foundation Trust and deployed to us, linking closely with our colleagues in Manchester Local Care Organisation. Our teams work as one and together we will make a massive difference to the health of people in Trafford.
We are looking for an enthusiastic and dynamic Community Flow Sister to join our community based Urgent Care team based at Wythenshawe hospital & Meadway Health Centre, with some hybrid working. Facilitating the smooth implementation of the patient DtA pathways, for people discharged to DtA from Hospital and therefore, requiring social care and health support on discharge from DtA. You will ensure that our co-ordinated and steam lined processes revolve around the patient and their needs and result in their safe discharge from DtA for our residents.
