Job summary
We are seeking an experienced GP with an interest in frailty and urgent community care for our newly developed Trafford Crisis Response service, part of Trafford Community Response (TCR). Join us and make a difference to people who would benefit from being cared for at home, to avoid unnecessary hospital admission. Providing a 7-day service over enhanced hours, we prevent admission to hospital in a crisis by wrapping integrated care around our patients to make them safe and stabilise in the home/community for 48hrs.
This is an exciting time to be part of our emerging integrated multi-disciplinary and multi-agency team providing specialist rapid assessment and interventions to our local population. As a GP with Extended Role (GPwER) in TCR, you will use your skills and experience in practising medicine holistically, managing risk and dealing with complexity and uncertainty, to provide holistic comprehensive assessment of patients with urgent medical needs and develop appropriate management plans to support patients to remain at home, wherever possible.
