Job summary
The post holder will be responsible for supporting the tracking of patients along cancer pathways to ensure all national and local cancer targets are met. The Cancer Tracker will be responsible for maintaining a cancer Patient Tracking List/s (PTLs) within a required tracking KPI. The Cancer Tracker will collect and record a wide range of cancer data as required ensuring the Trust meets the cancer agenda. The Cancer Tracker will be competent in utilising many of the Trust IT systems to access data and will use the Somerset Cancer Register (SCR), or other identified data platforms, to record all information on cancer patient tracking.
Main duties of the job
Are an integral part of the cancer services team, supporting the team in meeting all performance requirements set locally and nationally. Track patients throughout their journey, against the relevant cancer pathway(s) ensuring that patients are managed within the relevant national targets, liaising with the relevant ward/department. Promote awareness of target deadlines to relevant team members. Take appropriate action/escalate any delays with the relevant clinician, directorate management teams and cancer services management to prevent cancer target breaches. Support the Directorate and Cancer Team in management of patients and advising of patient transfers to enable the trust to provide accurate data for cancer waiting times. Attend any relevant tumour specific meetings e.g. PTLs. Collation of data from clinical reviews for patients whose wait is greater than 104 days (RCAs). Data collection to support the CWT Upload and any other national or local submissions.
