Job summary
This is an exciting opportunity for an enthusiastic, confident, well-motivated Nurse to join the Cardiac Rehabilitation team within the MFT cardiac single hospital service based at NMGH. The post holder must have sound cardiac knowledge, be currently working at senior band six or above and experienced in caring for patients eligible for Cardiac Rehabilitation with a keen interest in delivering health promotion and proven ability of leading on a project. This post in the first instance will be Thursday-Friday however some flexibility maybe possible. The role is based at NMGH but it is envisaged that there may be the opportunity to work across sites.
Main duties of the job
Support the Cardiac Rehabilitation Team in the organisation and management of multidisciplinary, patient centered care.Create and maintain effective communication with the multi-disciplinary team, patients, andrelatives.Undertake comprehensive health care needs assessment of patients, reassessing asappropriate.Undertake and promote practice sensitive to the needs of patients and relatives from multi-cultural backgrounds.
