Job summary

We are looking for an experienced and motivated Band 5 Staff Nurse to join our supportive, dynamic Manchester Community Response (MCR) Intermediate Care Unit in North Manchester.

The post will be based in our Intermediate Care Unit which is part of our Manchester Community Response Service which also includes the Crisis Response team, Home Pathway, Reablement, Community IV team, Pharmacy and Social care.

This is an exciting time to be part of our integrated multi-disciplinary and multi-agency service providing rehabilitation.

Main duties of the job

The main purpose of the role is to be responsible for the assessment of care needs, the planning, implementation and evaluation of nursing care within intermediate care.You will be supporting team members to deliver on their objectives through offering advice, guidance and support as appropriate with the focus being on delivering a high quality and efficient intermediate care service, to patients of all complexity, in a bedded intermediate care unit, including some work in patients home setting.You will be responsible for supervising junior staff and participating in the teaching of students.
