Roald Dahl Nurse Specialist for Children with Medical Complexity, Band 7 - Full Time

Are you looking for a new challenge? Do you want to make a difference to the lives of children and young people with medical complexity (CMC)? Royal Manchester Children's Hospital and Managed Clinical services (RMCH/MCS), in partnership with Roald Dahl's Marvelous Children's Charity, has an exciting and unique opportunity to work as a Roald Dahl CMC Nurse Specialist. As the role is the first of its kind nationally, this expert practitioner will be part of a new, innovative, and evolving area of paediatric care. In addition, you will receive support from Roald Dahl's Marvellous Children's Charity including training opportunities and peer support from the network of Roald Dahl Specialist.

The Roald Dahl CMC Nurse Specialist would enable proactive and inclusive services centered on the needs of the child and family to prevent hospitalisation, improve communication, improve resilience, enhance relationships between families and professionals, and to have greater information sharing across organisational boundaries.

Main duties of the job

Your main responsibilities will include:

Improving access to the CMC service

Coordination of care within the hospital setting and work with community teams to coordinate care to the point of discharge.

Work autonomously providing support for families/carers who are often in hospital for long periods of time.

Educate CYP and families / carers to learn new skills.

Providing review at outpatient appointments.

Provide clinical expertise to improve safe standards of care by active involvement in the patient journey and development of patient pathways.

Work collaboratively with all stakeholders to facilitate improved patient flow, capacity and continuity of care.

Reduce length of stay by assisting to streamline the patient hospital journey.

Prevention of hospital admission providing outpatient support.

Support for families/carers physical, emotional and social needs.

Support for transition to adult services.

Complete, and support ward staff to complete, CMC Healthcare passports.

Support the education of Trust staff regarding CMC care delivery.

Work with ACP to actively research and audit

A commitment to network with Roald Dahl Nurses already delivering specialist and holistic care

Collect data and compile accurate and up to date reports for both internal and external partners including Roald Dahl Marvellous Children's Charity.

Act as Ambassador for Roald Dahl Marvellous Children's Charity, raising awareness of the charity amongst colleagues and families.
