Job summary

The postholder will contribute to and be responsible for the day-to-day co-ordination and learning disability nursing support to children with significant learning disabilities and their families/carers.

The post holder will be required to work primarily in family homes but also in other settings that the child accesses.

The post holder will require good organisational skills, staff management skills, good reporting skills and relate effectively with children, families and other professionals both within and outside the organisation.

Main duties of the job

Deliver care that is person centred and underpinned by effective risk assessment and management e.g. safeguarding, positive handling

Undertake, contribute to and interpret specialist assessments as required e.g. functional analysis, risk assessments

Work with children and families on an individual basis to implement positive behavioural support interventions as formulated with the core team. Provide clinical and specialist advice concerning complex care packages e.g., behavioural support plans, continence promotion, hospital support, sleep interventions and the promotion of positive well-being.

Modelling and coaching behavioural techniques to parents/carers

Participate in and develop evidence-based specialist programmes of care whilst adopting a child centred approach. Evaluate individual care packages including recording analysis and presentation of relevant data (e.g. behaviour observation charts) to team members, including the use of IT to graphically present results from assessments and interventions.

Ensures that communication is based at a level that is understandable to each child and family and implement when necessary strategies to maximise communication with the child e.g. Picture Exchange Communication System, Objects of reference

Can communicate highly complex or sensitive/contentious information to colleagues and families in an empathic and professional manner
