Job summary
We are looking for enthusiastic Geriatricians with an interest in community working and transformation to join Hampshire and IOW healthcare NHS Foundation Trust.
This is an exciting time for this newly formed Trust as we redevelop our community services to match the needs of an ageing population and with the goal to move care closer to where our populations need it. There are opportunities across the Hampshire & IOW geographies: with vacancies in the Portsmouth and South East Division and the Isle of Wight. These are ideal posts for geriatricians who enjoy being hands on and working closely with colleagues in primary, secondary and community care. The job will be based from one of our community hubs; with time supporting inpatient wards, community assessment units, virtual wards and neighbourhood MDTs. There will be opportunities for additional SPA to be used towards developing the community offer or enhancing training and education opportunities.
Geriatricians are arguably the specialty most aligned with the future NHS model. And the future NHS model is community centred. We want to create a well-supported peer group of Geriatricians who will help us define the model, shape governance, set clinical standards and build the culture for innovative multidisciplinary working that happens outside hospital walls.
Main duties of the job
Hampshire and Isle of Wight Healthcare NHS Foundation Trust are seeking a Consultant Geriatrician to play a central role in the development and delivery of our community services on the Isle of Wight (IOW). The successful applicant will provide hybrid remote and in per-son cover across our community sites; St Mary's Hospital Community Unit, Ryde Health & Wellbeing Centre, Newport Health & Wellbeing Centre, South Locality teams (based in Sandown and Shanklin).
As an Organisation, HIOWH are investing in Community (Interface) Geriatricians as a vital part of our workforce; this role would link with peers across the different geographical localities.
A central component of the role will be medical advice and guidance, providing senior and experienced clinical oversight to complex and challenging patients across our community bedded care (14 bed Community Unit and 17 Intermediate Care beds in Springfield Nursing Home), the frailty virtual ward and Urgent Care Response. Day-to-day clinical responsibility for ward beds is provided by a multi-disciplinary team of nurses, therapists, a GP and Advance Clinical Practitioners. The consultant geriatrician is key to enabling the multi-disciplinary ward team to handle complexity, manage frailty expertly and help to widen our step-up capability for local patients. The post holders will work closely with acute Trust col-leagues to support appropriate treatment Escalation and step-down rehabilitation pathways.
