Job summary

The Rehabilitation Assistant will work as an active member of a community based team and will provide high quality, efficient and effective service to patients.

The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner. They will work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

The Rehabilitation Assistant will be proficient, knowledgeable and competent. They will develop in their role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.

Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.

Main duties of the job

Implement Therapy to a designated caseload.

Participation in treatment activity.

Dealing with distressing/sensitive issues.

To ensure patients and carers are an integral part of their treatment planning and goal setting.

To be responsible for issuing of standard aids, whilst demonstrating an awareness of local cost implications.

To ensure that up to date written and electronic records, reports and activity data are maintained in accordance with professional and Trust standards.

To provide intervention in a variety of environments, e.g. client's home, hospital/healthcare settings, schools, work environment to meet client needs.

To take part in procedures to ensure smooth running of the team such as patient registration, liaison reporting and recording data etc.

To adhere to national and local manual handling guidelines.

To support the work of the registered practitioners in the team. To prepare patients for and assist with clinical/therapeutic activities or treatments.

To provide a Therapy Service and develop continuity of care under the supervision of a Supervising Therapist within the laid down policies and procedures/code of ethics.

To be responsible for the health and safety and welfare of patients/clients and others, including the care/use of equipment provided.

To lone work adhering to local and HIOW lone working guidelines.

To work across sites and community settings
