Nurse Assessor - Memory Assessment Service

Job summary

We're looking for a dedicated Nurse Assessor join our Memory Assessment Service to support individuals and families navigating concerns around memory and cognitive health. Based in the heart of the Isle of Wight community, you'll play a key part in delivering timely assessments, person-centred care, and meaningful support at a pivotal moment in people's lives.

Main duties of the job

To work as part of the Memory Assessment Service frontline response to people referred for support and/or treatment. The post holder will complete nursing assessments to support the MDT to diagnose a dementia related illness and develop a plan of care post diagnosis, this may include identification of need in relation to medications, occupational therapy, cognitive stimulation therapy and signposting into community partners where appropriate.

The post holder will be a Registered Mental Health Nurse (RMN) with a minimum of two years' experience.

Essential

  • RMN or Allied health professional (Mental Health) - Evidence based and experience of reflective practice with a minimum of 2 years post registration.
  • Current national legislative and professional requirements relating to social and health care and processes implemented locally (The Care Act 2014, The Mental Capacity Act 2005, Mental Health Act 1983)
  • Mentorship /supervision qualifications/training

Experience

Essential

  • Assessment of risk, crisis management, intervention and management.
  • Wide knowledge/ appreciation of a range of models of practice in social and health care.
  • Knowledge of interventions and strategies which embrace social and health care needs.
  • Good communication and engagement skills, Solution Focused, Liaison/negotiation and Decision-making skills
