Job summary

Are you an experienced, highly skilled nurse passionate about delivering exceptional community based care? We are looking for a motivated Community Matron to support some of the most complex and vulnerable people in our communities.

In this senior clinical role, you will provide holistic assessments, personalised care planning and proactive management for individuals with long term conditions, frailty and complex health needs. Working autonomously, you will manage your own caseload, identify early signs of deterioration, and implement evidence based interventions to prevent avoidable hospital admissions.

You will play a key part in coordinating joined up care across GPs, District Nursing, Hospital at Home, mental health, social care and hospice teams. As a trusted point of contact for patients, carers and professionals, you will ensure seamless, integrated support that enhances patient experience and promotes independence.

You will also strengthen community nursing capacity through clinical leadership, supervision and education. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.
