We have a fantastic opportunity for an experienced Mental Health Practitioner to join our friendly, dynamic and busy multi-disciplinary team. Our team provides comprehensive assessment and intensive home treatment and delivers time specific, high quality and recovery focused care to adults experiencing a mental health crisis. We offer an alternative to hospital admission and support with early discharge and therefore work closely with our inpatient and community team colleagues to ensure an effective transition of care across the acute care pathway.

You can expect to work for a robust, well-established team with strong values in a positive working environment. As a team we prioritise staff wellbeing and progression and offer regular supervisions, reflective practice and in house education sessions.We offer a 24/7 service, 365 days a year.

Please note this is a part time position for 22.5 hours per week.

Main duties of the job

To ensure the provision of intensive community-based treatment, as an alternative to hospital admission.

To answer calls from service users of the crisis team and making calls to triage referrals and prompt medication where indicated although some shifts during the day may be required to provide home treatment, depending on the needs of the service.

To answer calls from the Mental Health Out of Hours advice line, document these appropriately and inform the relevant teams.

To review referrals from other agencies such as mental health liaison teams, emergency services, safe havens, out of hours GP and screen these appropriately.

To work closely with the inpatient units ensuring the effective transition of care across the acute pathway, this could involve some aspects of bed management and facilitating early discharges where possible.

To offer support, advice and input to services we work closely with such as the Safehaven Crisis Lounge and The Lookout Crisis House.

To work in a 24-hour, 7 day a week service, on a shift basis.

To proactively work in accordance with the principles of social inclusion, promoting independence and recovery.
