Job summary

If so, we'd love to welcome you to our evolving Community Mental Health Team as a Specialist Pharmacist. In this pivotal role, you'll help shape high quality, person centred care across our services, working alongside a dedicated multidisciplinary team to improve outcomes and champion best practice in mental health medicines optimisation.

Main duties of the job

The post-holder will provide highly specialist clinical, pharmaceutical and medicines optimisation expertise across Community Mental Health Services (CMHTs) and work closely across the wider Primary Care Networks (PCNs) on the Isle of Wight.

They will ensure the safe, effective, evidence-based and cost-efficient use of mental health medicines, support integrated care across organisations, and contribute to the development, delivery and evaluation of high-quality pharmacy services for people with mental health conditions.

Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

Qualifications

Essential

  • *Master of Pharmacy degree or equivalent.
  • *General Pharmaceutical Council registered pharmacist.
  • *Postgraduate Diploma (or MSc) in Clinical Pharmacy Practice or equivalent experience.

Desirable

  • *Member or associate member of the College of MH Pharmacy
  • *Diploma in Psychiatric Pharmacy
  • *Independent Prescriber qualification

Experience

Essential

  • *Substantial post-registration experience as a practicing pharmacist.
  • *Significant post-registration experience of mental health, community and hospital pharmacy.
  • *Multidisciplinary mental health team working and making clinically appropriate interventions.
  • *Clinical Audit.
  • *Training of other staff.
  • *Provision of information to patients and carers.

Desirable

  • *Demonstrable experience of cross sector collaborative working with primary care healthcare professionals including GPs.
  • *Experience in management of Mental Health patients
  • *Experience in clinical training and/or mentoring of pharmacy staff and other members of the MDT
  • *Service development
  • *Experience of in writing procedures, clinical guidelines and policy/protocols.
