Job summary
We are seeking a compassionate, skilled and motivated Co Occurring Conditions Mental Health Practitioner to join our Community Mental Health Team. This is an exciting opportunity to support individuals experiencing mental health difficulties alongside substance use, ensuring they receive high quality, person centered, integrated care.
You will play a key role in improving engagement, reducing barriers to treatment, and promoting recovery for people with complex and coexisting needs. Working across mental health, social care, and voluntary sector partners, you will help shape a consistent and trauma informed approach to cooccurring conditions across the local system.
Main duties of the job
The post holder will work autonomously to provide professional expertise in the assessment, planning, implementation, and evaluation of care with patients with co-occurring conditions.
To be responsible and accountable for ensuring the delivery of safe, effective, and evidence-based care for people allocated within the designated pathway.
To provide high quality care and treatment in conjunction with other professionals and work in partnership with other agencies to provide seamless, flexible services which meet the needs of patients and their carers to reach their care planned goals.
To ensure accurate and comprehensive assessments and risk management plans are in place.
To provide acute mental health ward in-reach support attending MDT reviews and supporting with safe transfer/discharge.
To signpost or refer service users to appropriate teams/agencies for appropriate treatment and support.
To contribute to training and development of colleagues within the team continue to deliver established workshops throughout the division.
To support the service in its implementation of clinical effectiveness programs by taking part in any agreed research, quality improvement projects and audit projects, leading on key aspects when identified as appropriate.
