Hampshire and Isle of Wight Healthcare NHS Foundation Trust

Rehabilitation Assistant

The closing date is 12 April 2026

Job summary

We are seeking a dedicated and compassionate Rehabilitation Assistant to join our Community Rehabilitation Team on the Isle of Wight.

In this role, you will support the delivery of high quality, multidisciplinary rehabilitation care across both NHS commissioned Community Rehab beds and patients' own homes.

Working under the delegation of Registered Nurses and Therapists, you will help patients regain independence, achieve personal goals, and transition safely back into their community.

This is a 7day service, offering a dynamic and rewarding environment where you will play a vital part in improving patient outcomes.

Main duties of the job

The successful candidate will deliver a range of delegated nursing and therapeutic interventions which focus on the direct rehabilitation needs of the patient under the care of the Community Rehabilitation Team.

Working within the boundaries set out by the Registered members of the Community Rehabilitation team (i.e. patients that have already had an initial assessment by the relevant staff members), the scope of the role enables the post holder to make accurate observations of a patient's ability, and feed this information back to the registered member of staff, in the following circumstances:

Washing and dressing

Meal preparation

Progression of patients with walking aids (crutches to walking stick for example)

Transferring from chair to bed etc and monitoring progress with this

Progression of patients with stair practice

Visiting patients at home and fitting of certain prescribed equipment

Bathing assessments post discharge of patients from Community Rehabilitation provider beds
