Job summary

We are seeking a motivated and forward thinking Registered Nurse to join our innovative South East Hampshire Community Nursing Team, specialising in Tissue Viability within a dynamic and supportive service.

In this role, you will contribute to the delivery of high quality specialist care across South East Hampshire, offering expert wound care and leg health guidance to patients, carers, and the wider multidisciplinary team.

You should bring experience in Tissue Viability--ideally within a community setting or from substantial acute practice--along with a strong commitment to ongoing professional development and expanding your expertise in wound management. Confidence in teaching and sharing specialist knowledge is important, as is the ability to work both autonomously and collaboratively within a multidisciplinary environment.

A valid UK driving licence and access to a vehicle for work purposes are required. Main duties of the job

The primary role of the Enhanced Wound Care Nurse is to work across a designated geographical area, providing support in the delivery of evidence based high quality enhanced wound/skin care, advice and education to a range of staff within the Integrated Community teams in their division.

The Enhanced Wound Care Nurse in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people with complex tissue viability issues they will be supported by a Tissue Viability Nurse Specialist.

The Enhanced Wound Care Nurse will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patient's own home as possible.

Review effectiveness of nursing care provided ensuring that patient safety is maintained and care delivery meets the standards required by the Trust and Care Quality Commission

Initiates policy and practice changes as a result of incidents, audits and complaints.

Provide management responsibility for the team in partnership with the integrated senior clinical team lead (Band 7). This may include annual leave management; sickness' / absence management; performance management, training and development, appraisals, incident and complaints management.

Deputise for the Integrated Senior Clinical Team Lead/s (Band 7) as required.

Provide clinical education and supervision in tissue viability for staff directed by the Tissue Viability Specialist Nurse.

Ensure that staff they line managing receive yearly appraisal and development plans in partnership with the Integrated Clinical lead. Proactive involvement in tissue viability audit in response to identified needs.

Provide proactive leadership for the team, ensuring they share a vision for their teams and organisation, and understand their role in achieving this.

Take an active role in promoting the interests and philosophy of the Department.

Ensure that equipment is used in the approved manner and is kept in good condition so that resources are used effectively and efficiently
