Job summary
The Intensive Support Team is a multi-disciplinary team that provides community-based assessment and interventions for adults with Learning Disabilities and challenging behaviours who require a period of intensive intervention. The Intensive Support Team uses the model of Positive Behaviour Support.
We are looking for a Therapy Assistant who will be proficient and knowledgeable in carrying out duties delegated to them by registered professionals within the multi-disciplinary team. The post holder will be expected to implement and evaluate therapeutic interventions in accordance with assessment recommendations. Develop excellent person-centred total communication skills and have the ability to confidently and quickly develop therapeutic relationships with service users in order to promote their participation and engagement in activities and improve their overall quality of life. An ability to develop and maintain professional relationships is a must to enable positive change to be effected for our service users. The postholder will be required to create a variety of person-centred accessible resources in addition to supporting the implementation and evaluation of Positive Behavioural Support strategies. They will work within a cohesive, knowledgeable and fun team of specialist clinicians from a range of disciplines. Strong IT and inter-personal skills are essential, as are a willingness to quickly learn new skills and be a strong team player.
Main duties of the job
Support the work of registered practitioners within the IST in line with the HIOW Learning Disability Clinical Competency Framework.
Complete assessments and outcome measures as delegated by registered practitioners.
Support the IST multidisciplinary team in completing assessments and planning Positive Behaviour Support (PBS) interventions.
Contribute to the development of PBS plans tailored to individuals in crisis or at risk of placement breakdown.
Trial and implement delegated interventions and PBS strategies, identifying and overcoming barriers to implementation.
Model PBS approaches to direct care staff and provide coaching, feedback and training on individual PBS plans.
Participate in Periodic Service Reviews to evaluate the effectiveness and quality of PBS plans.
Communicate information through easy read or audio visual resources using appropriate software and research tools in line with Trust policy.
Produce accessible resources such as communication books, visual schedules and accessible care plans.
Contribute to MDT discussions, decision making and meetings with service users, families and stakeholders.
Work collaboratively with families, carers and multiagency professionals.
Gather feedback from service users, families and carers to support service improvement.
Uphold safeguarding, Mental Capacity Act and Deprivation of Liberty Safeguards principles.
Act as a positive role model, upholding evidence based, high quality practice at all times.
